DESIGNED TO MEET YOUR NEEDS

ONE-SITE HEALTH CLINICS: Residents are encouraged to take advantage of reasonably priced on-site health clinics, hearing aid checks, flu and pneumonia vaccinations and education programs are all available without leaving home. Residents are responsible for direct payment or Medicare processing when applicable.

NEWSPAPER AND MAIL DELIVERY: Our mailboxes are conveniently located adjacent to the lobby. Your newspaper will be delivered to your apartment door daily.

TRANSPORTATION TO MEDICAL APPOINTMENTS: Scheduled transportation to medical appointments is provided within an 8 mile radius of our facility. Kaiser Sunnyside, Providence and seventh Day Adventist Hospitals and medical clinics are within our traveling distance. Appointments must be coordinated with our office staff. Medical transportation is available on both Tuesday mornings and all day Thursdays. Residents must be able to go unescorted to physician's office. There is no charge for scheduled medical transportation. Sorry, we cannot provide transportation for emergency medical services.

EXERCISE, EXERCISE: Join us three times each week for our range of motion exercise class, designed to keep your joints and muscles in shape. Most exercises are performed sitting down.

VIAL OF LIFE: When you complete your move-in materials, you will receive a medical history form. We urge you to store the competed form inside your medicine cabinet. Responding medical personnel will then have valuable information about your health care and medical care needs.

ANGEL RING: This is a voluntary program monitored by the residents of The Hazelwood. If you wish to participate, you simply hang your angel ring outside your apartment door when you retire at the end of the day. Remove the angel ring by 10am the next morning to signal your neighbors that you're "ok". If your angel ring is not removed by 10am, hall monitors will notify the office and we'll check to see that all is well.

SPECIAL DIETS: Our lunch and dinner menus will always offer a lower-sodium, low-fat option. Other dietary restrictions can be discussed with our Food Service Director.

COMPLIMENTARY WASHERS AND DRYERS: Washing machines and dryers are available for use at no charge on the second floor. Your linens and towels will be laundered by our Housekeeping staff every week as your apartment is cleaned. If you would like to have your personal laundry done for you, our staff can assist you in making arrangements with an outside provider at a very reasonable fee.

SPIRITUAL ENRICHMENT: Weekly religious service will be provided by area churches on a volunteer basis. Please refer to our Activities schedule.

IN-HOME COMPANIONS/PRIVATE PERSONAL AIDE: If you should require personal services outside the scope of those provided by our facility, our staff can assist you in finding just the right combination of help you need. The cost of this service is not included in your monthly rent.

SPECIAL MOBILITY TRANSPORTATION: Our staff will happily assist you in making transportation arrangements with public special transportation providers.

CABLE TV: Basic cable television is provided at no charge. If you wish for expanded service, our staff can assist you in making these arrangements.

AIR CONDITIONING: Your apartment has personally controlled heat and air conditioning.

ABOVE ALL: We remain true to our values by providing the best possible services to our residents. We also believe in keeping a light, warm and friendly spirit. We work hard to bring our residents excellent service, but we also like to smile, laugh and have fun.
